SPEAKER VIDEOS ORDER FORM
Instructions:
- Print out this form.
- Fill in the address you would like the videos sent to.
- Indicate the name of the speaker and part number.
- Cost is $39.95 plus $3.50 shipping and handling (total $43.45) for each video.
- Make checks payable to "Golden Gate Wing".
- Mail this form and your check to the Wing address below.
- Please note that shipment of your video order can take several weeks and will be on DVD (unless specified otherwise). The Wing staff meets once per month, which is when Wing mail is handed out. Then a Wing member has to schedule time to pull the Video, make the duplication, create the labels, package it and mail it to you. We are all volunteers and we hope you understand that this can take time to accomplish.
